A guarantee of payment (GOP) is a document your travel
insurer’s assistance company sends to a Bali hospital promising to pay
your bill, which lets the hospital treat you cashlessly instead of
making you pay upfront.
To get one, you (or a coordinator) must
call your insurer’s 24-hour emergency assistance line early, provide
your policy and incident details, and let them liaise with the
hospital’s billing office. The GOP is the mechanism that turns “pay a
deposit now” into “your insurer pays directly.” Speed matters — the
earlier the GOP is requested, the smoother admission is.

If you have never dealt with international travel insurance in a

What a guarantee of
payment actually is

A guarantee of payment — often called a GOP or a “guarantee letter” —
is a formal commitment from your insurer (or, more precisely, their
appointed medical assistance company) to the hospital, confirming that
they will cover the eligible costs of your treatment up to a stated
amount. With a GOP in hand, the hospital knows it will be paid and can
therefore proceed cashless: you are not asked to fund
treatment from your own card and reclaim it later.

Without a GOP, the hospital’s default position is understandable
caution — they ask you for a deposit or full payment, and you
deal with your insurer afterwards through
reimbursement. The GOP is what flips that dynamic.

How to get a GOP: the
sequence

The process follows a fairly consistent path:

  1. Call your insurer’s 24-hour emergency assistance
    line
    as soon as a significant admission looks likely. The
    number is on your policy document or membership card. This is not the
    general claims line — it is the emergency/medical assistance line, which
    can act in real time.
  2. Provide the essentials: policy number, patient
    details, the hospital name, the treating doctor, and a description of
    the condition.
  3. The assistance company verifies cover and, if the
    treatment is eligible, contacts the hospital’s international or billing
    desk.
  4. They issue the GOP to the hospital, stating the
    amount guaranteed.
  5. The hospital switches you to cashless billing for
    covered costs.

The reason coordinators exist is that steps 1–4 are genuinely hard to
drive when you are ill, shocked, or watching over a loved one. Making a
clear, persistent case to an assistance company across time zones is a
job in itself.

Timing is everything

A GOP is far easier to arrange before or at
admission
than after a large bill has built up. Insurers are
more comfortable guaranteeing a defined, upcoming treatment than
retroactively covering costs already incurred. If you wait until
discharge, you may be forced into reimbursement even where cashless was
